Introduction

This Private Clinical Concierge Engagement Agreement (“Agreement”) is entered into

between Magnolia Medical Companion (“Company”) and the undersigned Client and/or

Responsible Party (“Client”).

1. Nature of Services

Magnolia Medical Companion provides personalized clinical concierge support including

accompaniment, note-taking, care coordination, and logistical support. All services are non-

medical and do not replace licensed healthcare providers.

2. Elevated Standard of Care

Services are delivered with discretion, professionalism, and strict confidentiality.

3. Fees & Retainer Structure

Services are private-pay. Retainers are non-refundable once services begin. Payment is due

as outlined in the Service & Investment Guide.

4. Scheduling & Cancellations

A minimum of 24 hours notice is required. Late cancellations may be billed in full.

5. Confidentiality & Authorization

All client information is treated as confidential. HIPAA authorization is required for

communication with healthcare providers.

6. Limitation of Liability

Magnolia Medical Companion is not responsible for medical outcomes, provider decisions,

or facility actions. Liability is limited to fees paid.

7. Client Responsibilities

The Client agrees to provide accurate information and remains responsible for all medical

decisions.

8. Term & Termination

This Agreement remains in effect until terminated. Outstanding balances are due upon

termination.

9. Governing Law

This Agreement is governed by the laws of the State of Tennessee.

10. Entire Agreement

By requesting services from Magnolia Medical Companion, you acknowledge that you have read, understood, and agree to the terms outlined in this agreement.
